Patient Care Coordinator

Remote (United States)

Role overview

Patient Care Coordinator connects patients with psychiatric care by managing referrals in both directions. As the first contact for people referred by partner organizations, the role combines patient outreach, scheduling, administrative coordination, and persuasive communication. It also coordinates referrals to outside organizations when patients need services beyond the practice's scope, keeping patients, providers, and receiving partners informed throughout the process.

Patient Outreach and Scheduling

  • Initiate outreach for referrals that have not yet been scheduled and record outcomes accurately.
  • Handle a high volume of outbound calls in a fast-paced environment.
  • Reach patients by phone, text, and email to explain next steps, communicate the value of psychiatric care, and help them schedule appointments.
  • Use motivational interviewing to address concerns, establish urgency, and encourage patients to book psychiatric evaluations.
  • Use internal scheduling tools to match patients with psychiatrists according to clinical needs, geography, and insurance.
  • Send self-scheduling links, follow up with patients, and resolve barriers such as insurance questions or appointment availability.

Outbound Referral Coordination

  • Coordinate referrals to partner organizations for higher levels of care, specialized programs, community resources, and other services outside the practice's scope.
  • Explain the reason for each referral, what the patient can expect, and how to connect with the receiving organization.
  • Confirm that partners receive referrals, provide required documentation, and monitor progress through intake.
  • Update the referring provider on receipt confirmations, scheduling outcomes, and barriers.
  • Follow up with patients and partner organizations until referrals are resolved, reducing gaps between care providers.
  • Document all outbound referral activity in internal systems to maintain a clear audit trail for the care team.

Qualifications

  • One to three years of experience in healthcare coordination, patient intake, a call center, or referrals.
  • Clear written and verbal communication, a strong phone presence, empathy, and a persuasive communication style.
  • Comfort working with CRMs, EHRs such as eClinicalWorks, and ticketing systems such as Zendesk and ServiceNow.
  • The ability to multitask and work efficiently with a high volume of requests.
  • Strong organization, responsiveness, attention to detail, and consistent follow-through.

Preferred experience

  • Experience coordinating care or referrals across multiple organizations or provider networks is a plus.
  • Sales or customer retention experience, including upselling, rebooking, or high-volume conversion work, is strongly preferred.
